Os fatores respons\u00e1veis pela r\u00e1pida expans\u00e3o das arboviroses no ocidente ainda est\u00e3o sob investiga\u00e7\u00e3o, mas os principais deles est\u00e3o fortemente ligados \u00e0 a\u00e7\u00e3o humana, como o desmatamento, falta de saneamento b\u00e1sico, migra\u00e7\u00e3o e urbaniza\u00e7\u00e3o, sem contar as mudan\u00e7as clim\u00e1ticas. dir=\"auto\"><span class=\"akGp8 SLuLS\">\u00a0 \u00a0 \u00a0 \u00a0 \u00a0 A febre do Mayaro, assim como a chikungunya, \u00e9 causada por um v\u00edrus de fita simples de RNA envelopado pertencente ao g\u00eanero <em>Alphavirus, <\/em>da fam\u00edlia <em>Togaviridae<\/em>. Esse v\u00edrus \u00e9 transmitido principalmente por mosquitos do g\u00eanero <em>Haemagogus<\/em> entre primatas n\u00e3o humanos que habitam florestas tropicais, mas experimentos demonstram que o mosquito urbano <em>Aedes aegypti <\/em>tamb\u00e9m \u00e9 capaz de transport\u00e1-lo. Por isso, apesar de poucas evid\u00eancias que sustentem um ciclo urbano, existe uma preocupa\u00e7\u00e3o de que o v\u00edrus possa utilizar o <em>Aedes<\/em> como vetor, infectando tamb\u00e9m os seres humanos na cidade, como j\u00e1 \u00e9 o caso do v\u00edrus da febre Oropouche, que apresenta um ciclo silvestre e um ciclo urbano[1].<\/span><\/p><\/div><div data-breakout=\"normal\"><p id=\"viewer-12jno\" class=\"DFnp0 WloJR SLuLS _6qLM3\" dir=\"auto\"><span class=\"akGp8 SLuLS\"> \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 Em 2016, foi detectada em um menino de 8 anos uma coinfec\u00e7\u00e3o pelos v\u00edrus da dengue e da febre do Mayaro. O paciente se encontrava em uma \u00e1rea semi rural no Haiti, a cerca de 32 km da capital. O contexto desse caso sugere que talvez o mosquito <em>Aedes aegypti <\/em>possa ter sido o agente respons\u00e1vel pela transmiss\u00e3o de ambos os v\u00edrus ao mesmo tempo. Al\u00e9m disso, o fato de o Haiti n\u00e3o abrigar esp\u00e9cies nativas de primatas sugere a ocorr\u00eancia de outro animal como reservat\u00f3rio para o v\u00edrus da febre do Mayaro, como talvez o pr\u00f3prio ser humano. O v\u00edrus da febre do Mayaro j\u00e1 foi tamb\u00e9m detectado uma vez em aves, apesar de a sua transmiss\u00e3o nesses animais ainda n\u00e3o ter sido muito bem estudada. Contudo, esse pode ser mais um indicativo da adapta\u00e7\u00e3o do v\u00edrus para infectar diferentes grupos [4].<\/span><\/p><\/div><div data-breakout=\"normal\"><p id=\"viewer-ee9on\" class=\"DFnp0 WloJR SLuLS _6qLM3\" dir=\"auto\"><span class=\"akGp8 SLuLS\"> \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 Pesquisadores da USP sugerem que tanto o v\u00edrus da febre do Mayaro quanto o da Chikungunya podem sofrer muta\u00e7\u00f5es e se adaptar a novos ciclos zoon\u00f3ticos de transmiss\u00e3o, podendo assim representar riscos mais altos de gerar epidemias. Essas hip\u00f3teses requerem mais investiga\u00e7\u00f5es, mas uma das dificuldades est\u00e1 nos diagn\u00f3sticos das doen\u00e7as.<\/span><\/p><\/div>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"has_eae_slider elementor-element elementor-element-f9f69a6 e-flex e-con-boxed wpr-particle-no wpr-jarallax-no wpr-parallax-no wpr-sticky-section-no wpr-equal-height-no e-con e-parent\" data-eae-slider=\"31727\" data-id=\"f9f69a6\" data-element_type=\"container\" data-e-type=\"container\">\n\t\t\t\t\t<div class=\"e-con-inner\">\n\t\t\t\t<div class=\"elementor-element elementor-element-fffeeeb elementor-widget elementor-widget-text-editor\" data-id=\"fffeeeb\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<div data-breakout=\"normal\"><h3 id=\"viewer-aqa4\" class=\"Tu1-z AYDy0 nsGWt _6qLM3\" dir=\"auto\"><span class=\"qKwSW\"><strong>Febre Oropouche<\/strong><\/span><\/h3><\/div><div data-hook=\"rcv-block13\">\u00a0 \u00a0 \u00a0 \u00a0 A febre Oropouche \u00e9 transmitida por um v\u00edrus pertencente a uma fam\u00edlia diferente, chamada <em>Bunyaviridae<\/em>. No ciclo urbano, seu principal vetor \u00e9 o <em>Culicoides paraensis<\/em>, pertencente a uma fam\u00edlia de mosquitos conhecidos popularmente em alguns lugares como &#8220;maruins&#8221; ou &#8220;mosquitos-do-mangue&#8221;, mas tamb\u00e9m pode ser transmitido por outros agentes, como os mosquitos <em>Culex quinquefasciatus<\/em>, <em> Ochlerotatus serratus <\/em>e inclusive pelo <em>Aedes aegypti.<\/em> Tanto os mosquitos do g\u00eanero <em>Culicoides <\/em>quanto o <em>Culex quinquefasciatus <\/em>se alimentam de sangue de outros animais al\u00e9m dos humanos, sendo os <em>Culicoides<\/em> ainda vetores de arboviroses em equinos e bovinos. No ciclo silvestre, o v\u00edrus da febre Oropouche circula entre bichos-pregui\u00e7a e diversas esp\u00e9cies de macacos [10].<\/div><div data-breakout=\"normal\"><p id=\"viewer-571pb\" class=\"DFnp0 WloJR SLuLS _6qLM3\" dir=\"auto\"><span class=\"akGp8 SLuLS\">\u00a0 \u00a0 \u00a0 \u00a0 \u00a0 Alguns pesquisadores apontam a febre Oropouche como uma das arboviroses mais frequentes no Brasil, atr\u00e1s apenas da Dengue, sendo uma doen\u00e7a emergente negligenciada. Sua introdu\u00e7\u00e3o no Brasil se deu nos anos 60, na regi\u00e3o norte do pa\u00eds, e desde ent\u00e3o h\u00e1 ocorr\u00eancias de surtos epid\u00eamicos peri\u00f3dicos em certas regi\u00f5es end\u00eamicas da doen\u00e7a [3].<\/span><\/p><\/div>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"has_eae_slider elementor-element elementor-element-ded9bb4 e-flex e-con-boxed wpr-particle-no wpr-jarallax-no wpr-parallax-no wpr-sticky-section-no wpr-equal-height-no e-con e-parent\" data-eae-slider=\"42085\" data-id=\"ded9bb4\" data-element_type=\"container\" data-e-type=\"container\">\n\t\t\t\t\t<div class=\"e-con-inner\">\n\t\t\t\t<div class=\"elementor-element elementor-element-b2a1195 elementor-widget elementor-widget-text-editor\" data-id=\"b2a1195\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<div data-breakout=\"normal\"><h3 id=\"viewer-ei446\" class=\"Tu1-z AYDy0 nsGWt _6qLM3\" dir=\"auto\"><span class=\"qKwSW\"><strong>Epidemiologia<\/strong><\/span><\/h3><\/div><div data-breakout=\"normal\"><p id=\"viewer-eof4a\" class=\"DFnp0 WloJR SLuLS _6qLM3\" dir=\"auto\"><span class=\"akGp8 SLuLS\">\u00a0 \u00a0 \u00a0 Devido aos sintomas dessas duas doen\u00e7as serem semelhantes aos de outras arboviroses mais frequentes e aos diagn\u00f3sticos serem principalmente cl\u00ednicos, \u00e9 prov\u00e1vel que elas estejam sendo subnotificadas e que, portanto, o n\u00famero de casos seja maior do que se tem registro atualmente [2,3,4,9]. Al\u00e9m disso, a similaridade do v\u00edrus da febre do Mayaro com o da chikungunya pode gerar rea\u00e7\u00f5es cruzadas de imunidade no paciente, o que acaba confundindo at\u00e9 mesmo os testes sorol\u00f3gicos, pois anticorpos similares s\u00e3o produzidos pelo organismo em resposta aos dois v\u00edrus, j\u00e1 que suas estruturas s\u00e3o muito semelhantes [2,11]. Tendo isso em vista, as Secretarias de Sa\u00fade de alguns estados do norte do Brasil v\u00eam alertando os munic\u00edpios para a import\u00e2ncia de aumentar o monitoramento e a precis\u00e3o no diagn\u00f3stico das doen\u00e7as.<\/span><\/p><\/div><div data-breakout=\"normal\"><p id=\"viewer-faitr\" class=\"DFnp0 WloJR SLuLS _6qLM3\" dir=\"auto\"><span class=\"akGp8 SLuLS\"> \u00a0 \u00a0 \u00a0 Uma pesquisa realizada em Manaus pela Funda\u00e7\u00e3o de Medicina Tropical Dr. Heitor Vieira Dourado (FMT-HVD) revelou um surto silencioso da febre de Oropouche entre 2007 e 2008, com 128 casos confirmados pelos testes laboratoriais de sorologia, de um total de 631 pacientes testados, que nos exames cl\u00ednicos haviam sido diagnosticados como dengue. Para a febre de Mayaro, foram confirmados outros 33 desse total [9]. A fim de aumentar a acur\u00e1cia de diagn\u00f3stico dessas arboviroses, foi desenvolvido pelo Instituto Le\u00f4nidas &amp; Maria Deane (ILMD\/Fiocruz Amazonas) um protocolo que possibilita o diagn\u00f3stico molecular da infec\u00e7\u00e3o pelos v\u00edrus da febre do Mayaro e\/ou Oropouche, de maneira simult\u00e2nea, com alta sensibilidade e especificidade, utilizando a t\u00e9cnica de PCR em Tempo Real [8]. O m\u00e9todo j\u00e1 est\u00e1 em uso, mas atualmente a confirma\u00e7\u00e3o laboratorial por t\u00e9cnicas de biologia molecular est\u00e1 dispon\u00edvel apenas em institui\u00e7\u00f5es que realizam pesquisa no Brasil. Entre 1\u00ba de janeiro e 10 de abril deste ano (2023), o Laborat\u00f3rio Central de Sa\u00fade P\u00fablica de Roraima (Lacen-RR) analisou amostras de 320 pacientes, incorporando no seu escopo de exames o diagn\u00f3stico molecular, e dentre elas 26 foram positivas para Oropouche e uma para Mayaro [5].<\/span><\/p><\/div><div data-breakout=\"normal\"><p id=\"viewer-2veek\" class=\"DFnp0 WloJR SLuLS _6qLM3\" dir=\"auto\"><span class=\"akGp8 SLuLS\"> \u00a0 \u00a0 \u00a0 \u00a0Outros esfor\u00e7os voltados para essa \u00e1rea de pesquisa s\u00e3o importantes para o maior entendimento das arboviroses e, consequentemente, tornam-se pe\u00e7a essencial para o combate dessas doen\u00e7as, como o projeto Zibra: <em>mapeamento gen\u00e9tico do Zika e outros arbov\u00edrus no Brasil, <\/em>que percorre as regi\u00f5es brasileiras para realizar o sequenciamento gen\u00e9tico de v\u00edrus de arboviroses a partir de amostras de pacientes de diversos munic\u00edpios. Em 2019, o levantamento foi feito na regi\u00e3o Centro-Oeste do pa\u00eds em parceria com laborat\u00f3rios de sa\u00fade p\u00fablica do Minist\u00e9rio da Sa\u00fade, e os Laborat\u00f3rios Centrais de Sa\u00fade P\u00fablica (LACEN) dos quatro estados. A Secretaria de Sa\u00fade de Goi\u00e1s publicou uma not\u00edcia acerca dessa parceria onde colocam, como um dos objetivos do projeto, o sequenciamento de 400 amostras que foram positivas para pelo menos um dos oito v\u00edrus transmitidos (incluindo os da febre do Mayaro e Oropouche) pelos mosquitos mais relevantes para a regi\u00e3o.
