Desde a industrializa\u00e7\u00e3o e o aumento da urbaniza\u00e7\u00e3o da popula\u00e7\u00e3o, as condi\u00e7\u00f5es de sa\u00fade coletiva e de sa\u00fade p\u00fablica t\u00eam apresentado situa\u00e7\u00f5es cada vez mais complexas, especialmente nos pa\u00edses em desenvolvimento. As condi\u00e7\u00f5es naturais do ambiente f\u00edsico &#8211; especialmente o clima &#8211; associadas ao contexto sociocultural e \u00e0 inefic\u00e1cia das pol\u00edticas p\u00fablicas de sa\u00fade promovem o desenvolvimento e prolifera\u00e7\u00e3o dos focos de mosquitos urbanos, resultando em epidemias graves. Assim, para entender as quest\u00f5es de dispers\u00e3o e transmiss\u00e3o de dengue, \u00e9 necess\u00e1rio abordar n\u00e3o somente o comportamento dos indiv\u00edduos infectados, mas a correla\u00e7\u00e3o entre todas as caracter\u00edsticas geogr\u00e1ficas e clim\u00e1ticas presentes no meio. O estudo que apresentamos nesse resumo teve como foco a an\u00e1lise do clima urbano e da prolifera\u00e7\u00e3o da dengue em tr\u00eas diferentes cidades no Brasil: Campo Grande (MS), Maring\u00e1 (PR) e Ribeir\u00e3o Preto (SP), correlacionando as vari\u00e1veis clim\u00e1ticas e a incid\u00eancia da dengue por meio da utiliza\u00e7\u00e3o do GIS (Sistema de Informa\u00e7\u00e3o Geogr\u00e1fica) e outras ferramentas para entender as din\u00e2micas do clima urbano.<\/span><\/p><\/div><div data-breakout=\"normal\"><p id=\"viewer-3lahk\" class=\"DFnp0 WloJR SLuLS _6qLM3\" dir=\"auto\"><span class=\"akGp8 SLuLS\"> \u00a0 \u00a0 \u00a0 O arbov\u00edrus causador da dengue \u00e9 o mais significativo entre os que afetam humanos e, globalmente, em torno 2,5 bilh\u00f5es de pessoas vivem em \u00e1reas onde esses v\u00edrus podem ser transmitidos. Ele pertence ao g\u00eanero <em>Flavivirus<\/em>, da fam\u00edlia <em>Flaviviridae<\/em>, e sua infec\u00e7\u00e3o \u00e9 causada por quatro sorotipos que produzem imunidade espec\u00edfica para sorotipos. A infec\u00e7\u00e3o causada por um dos quatro sorotipos torna o indiv\u00edduo permanentemente imune ao respectivo sorotipo, mas n\u00e3o confere imunidade cruzada para os demais; isso significa que uma pessoa vivendo em uma regi\u00e3o onde os quatro sorotipos encontram-se em circula\u00e7\u00e3o pode contrair dengue at\u00e9 quatro vezes em sua vida e tamb\u00e9m que as epidemias de dengue podem ser influenciadas pelos sorotipos ocorrentes em uma regi\u00e3o. O principal vetor da doen\u00e7a no Brasil \u00e9 o <em>Aedes aegypti, <\/em>normalmente encontrado em \u00e1reas urbanas, enquanto o <em>Aedes albopictus<\/em> \u00e9 mais frequente em \u00e1reas rurais. A temperatura e a precipita\u00e7\u00e3o interferem diretamente no ciclo de vida desses mosquitos<em>, <\/em>determinando suas taxas de desenvolvimento e reprodu\u00e7\u00e3o. Al\u00e9m disso, fatores urbanos como a aus\u00eancia de coleta adequada de lixo, casas abandonadas ou piscinas malconservadas acentuam ainda mais o problema.<\/span><\/p><\/div><div data-breakout=\"normal\"><p id=\"viewer-7car\" class=\"DFnp0 WloJR SLuLS _6qLM3\" dir=\"auto\"><span class=\"akGp8 SLuLS\"> \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 As tr\u00eas cidades foco do estudo, tem climas semelhantes, altas taxas de doen\u00e7as e um hist\u00f3rico de s\u00e9rias epidemias. Localizam-se na regi\u00e3o centro-sul do Brasil e s\u00e3o importantes centros de transporte, onde ocorre um grande fluxo de pessoas e bens, o que nos permite estudar a dimens\u00e3o da import\u00e2ncia de fatores socioecon\u00f4micos em rela\u00e7\u00e3o \u00e0 transmiss\u00e3o de doen\u00e7as. Seu \u00edndice de desenvolvimento humano (IDH) evoluiu significativamente nos \u00faltimos 20 anos, mas ainda assim, epidemias de dengue significativas foram registradas de 2000 a 2011.<\/span><\/p><\/div><div data-breakout=\"normal\"><p id=\"viewer-e9svk\" class=\"DFnp0 WloJR SLuLS _6qLM3\" dir=\"auto\"><span class=\"akGp8 SLuLS\"> \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 No ver\u00e3o, que \u00e9 um per\u00edodo bastante chuvoso e quando o ambiente urbano \u00e9 quente as cidades formam ambientes altamente favor\u00e1veis \u200b\u200b\u00e0 prolifera\u00e7\u00e3o do vetor da dengue. Entre janeiro e abril os maiores \u00edndices de dengue foram registrados nas tr\u00eas cidades, onde a forte influ\u00eancia do clima na forma\u00e7\u00e3o de epidemias p\u00f4de ser observada, al\u00e9m das condi\u00e7\u00f5es de vida das popula\u00e7\u00f5es e a din\u00e2mica de urbaniza\u00e7\u00e3o.<\/span><\/p><\/div><div data-breakout=\"normal\"><p id=\"viewer-5b556\" class=\"DFnp0 WloJR SLuLS _6qLM3\" dir=\"auto\"><span class=\"akGp8 SLuLS\"> \u00a0 \u00a0 \u00a0 \u00a0 \u00a0 Ao analisar a incid\u00eancia de dengue em <strong>escalas de tempo sazonais <\/strong>(esta\u00e7\u00f5es ao longo dos anos) os autores verificaram que a ocorr\u00eancia de outros sorotipos gera novas epidemias devido \u00e0 falta de imunidade na popula\u00e7\u00e3o. Esse fen\u00f4meno \u00e9 observ\u00e1vel em todas as cidades analisadas, mas com um atraso de um m\u00eas para o pico de epidemia em Campo Grande (fevereiro) e tr\u00eas meses para Maring\u00e1 e Ribeir\u00e3o Preto (abril), talvez explicado pelas diferen\u00e7as entre o in\u00edcio e o fim das esta\u00e7\u00f5es chuvosas em cada cidade. Os maiores valores de precipita\u00e7\u00e3o normalmente coincidem com as maiores epidemias que ocorreram &#8211; em Campo Grande e Maring\u00e1, a ocorr\u00eancia de suas maiores epidemias coincidiu com anos de precipita\u00e7\u00e3o acumulada de 500\/600 mm nos tr\u00eas meses antes das epidemias. Desse modo, previs\u00f5es clim\u00e1ticas sazonais poderiam ajudar a prever meses de incid\u00eancia de dengue antecipadamente no sudeste brasileiro.<\/span><\/p><\/div><div data-breakout=\"normal\"><p id=\"viewer-e7a0u\" class=\"DFnp0 WloJR SLuLS _6qLM3\" dir=\"auto\"><span class=\"akGp8 SLuLS\"> \u00a0 \u00a0 \u00a0 \u00a0 \u00a0A figura 2 representa a an\u00e1lise espacial de anomalias nos per\u00edodos de esta\u00e7\u00f5es chuvosas (dezembro a mar\u00e7o) durante duas importantes epidemias e um ano de n\u00e3o epidemia nas tr\u00eas cidades estudadas. A an\u00e1lise mostra que deve haver uma boa taxa de precipita\u00e7\u00e3o na regi\u00e3o durante o per\u00edodo de dezembro a mar\u00e7o para que ocorram epidemias, mas, mesmo assim a epidemia pode n\u00e3o ocorrer.<\/p><p class=\"DFnp0 WloJR SLuLS _6qLM3\" dir=\"auto\"><span class=\"akGp8 SLuLS\">\u00a0 \u00a0 \u00a0 \u00a0 \u00a0 <\/span>Em Ribeir\u00e3o Preto, foi observada uma grande epidemia em 2010 ap\u00f3s anomalias positivas de precipita\u00e7\u00e3o e a re-introdu\u00e7\u00e3o do sorotipo Den-1. Contudo, a epidemia em 2011 foi menor que em 2010, provavelmente porque a precipita\u00e7\u00e3o total excedeu a faixa ideal, especialmente durante mar\u00e7o, quando fortes chuvas podem ter lavado os criadouros. Durante o per\u00edodo, nenhuma correla\u00e7\u00e3o significativa entre anomalias positivas de temperatura e aumento dos casos de dengue foi encontrada. Quando avaliada a <strong style=\"color: #333333\">escala de tempo di\u00e1ria<\/strong> percebe-se uma clara rela\u00e7\u00e3o entre temperatura e casos de dengue. Essa an\u00e1lise demonstra um intervalo de tempo de aproximadamente sete dias entre a mudan\u00e7a de temperatura e a ocorr\u00eancia da doen\u00e7a. Em Maring\u00e1 a redu\u00e7\u00e3o da temperatura na metade de mar\u00e7o levou \u00e0 estabiliza\u00e7\u00e3o do crescente n\u00famero de casos de dengue na cidade. Logo em seguida, os casos da doen\u00e7a voltaram a aumentar conforme a temperatura subia novamente. Essas descobertas confirmam estudos anteriores, nos quais j\u00e1 havia sido relatada a import\u00e2ncia da temperatura no ciclo de vida e na atividade do mosquito em uma escala temporal menor.<\/p><\/div><div data-hook=\"rcv-block37\"><strong style=\"color: #333333\"><span class=\"akGp8 SLuLS\">\u00a0 \u00a0 \u00a0 \u00a0 \u00a0 <\/span>Os resultados deste estudo demonstram a import\u00e2ncia dos fatores clim\u00e1ticos (precipita\u00e7\u00e3o e temperatura) e dos sorotipos na ocorr\u00eancia de epidemias<\/strong>. Contudo, vale ressaltar que as condi\u00e7\u00f5es clim\u00e1ticas, assim como o perfil sorot\u00edpico, n\u00e3o s\u00e3o os \u00fanicos fatores respons\u00e1veis \u200b\u200bpela evolu\u00e7\u00e3o temporal da dengue. O status de imunidade da popula\u00e7\u00e3o e elementos sociais e ambientais tamb\u00e9m devem ser considerados. Al\u00e9m disso, pol\u00edticas p\u00fablicas locais e a ades\u00e3o a elas desempenham um papel importante na manifesta\u00e7\u00e3o da dengue e controle da doen\u00e7a.<\/div><div data-breakout=\"normal\"><p id=\"viewer-a0mde\" class=\"DFnp0 WloJR SLuLS _6qLM3\" dir=\"auto\"><span class=\"akGp8 SLuLS\">\u00a0 \u00a0 \u00a0 \u00a0 \u00a0 O estilo de vida dos habitantes tamb\u00e9m pode ser considerado uma condi\u00e7\u00e3o para a manifesta\u00e7\u00e3o da dengue. Em particular, o descarte de lixo e entulho de constru\u00e7\u00e3o deixados nos quintais das casas ou piscinas abandonadas podem servir de reservat\u00f3rio para reprodu\u00e7\u00e3o de mosquitos, situa\u00e7\u00e3o que foi observada em v\u00e1rios locais dentro das tr\u00eas cidades. Outro fator importante que favorece a circula\u00e7\u00e3o de sorotipos e o aumento de casos de dengue \u00e9 a mobilidade da popula\u00e7\u00e3o (dentro da cidade e entre elas). A maioria das notifica\u00e7\u00f5es de casos importados nas tr\u00eas cidades vem de pequenas cidades circunvizinhas que usam os servi\u00e7os de sa\u00fade e assist\u00eancia delas.<\/span><\/p><\/div><div data-breakout=\"normal\"><p id=\"viewer-a90qg\" class=\"DFnp0 WloJR SLuLS _6qLM3\" dir=\"auto\"><span class=\"akGp8 SLuLS\">\u00a0 \u00a0 \u00a0Por se tratar de um assunto de natureza complexa, a quest\u00e3o da dengue requer solu\u00e7\u00f5es interdisciplinares e o estudo que apresentamos aqui \u00e9 um exemplo, pois para ser constru\u00eddo exigiu a intera\u00e7\u00e3o entre profissionais das \u00e1reas da sa\u00fade, clima e geografia.
